may be entitled to Social Security Disability benefits. In any Social Security
Disability case, documentation is the key. It is important to be able to show the
Social Security Administration medical records that reveal why your doctors feel
you are limited physically or mentally.

The initial application for Social Security Disability benefits can be done on your
